Even Walking Is Hazardous, Given the “Right” Conditions

A motorman was attempting to cross a 12” tall berm and plastic liner to reach the diesel tank area when he slipped and fell into a 16” deep production containment area, breaking both bones in his leg.

In this instance, a dedicated walking surface across the pit liner had not been defined and the surface was slippery from rain. Workers had removed the grating from the top of the production piping enclosure the week before and failed to replace it.

While the immediate solution is to address these specific problems, the accident serves as a reminder that the simplest activities can result in injury. Walking areas should be examined on a routine basis for possible hazards; these include walkways, stairs and working surfaces.

Especially as operations are increasing their use of plastic liners and berms to mitigate potential environmental exposures, personnel need to be reminded to be careful. Walkways should be installed over berms, including properly constructed stairs.

It goes without saying that everyone on the site should be vigilant – and take action – where they see actions that are hazardous to personnel, such as the missing grate.
