Rig Crane Collapse Kills Two

An incident involving a rig crane recently resulted in two fatalities.

The rig crew was installing ropes for the jack-up rig in anticipation of a re-entry into the well. The crane was lifting a personnel basket containing one person tied off to the basket with fall protection. As the crane was booming up, the boom failed. The boom fell to the deck and the big block fell on one employee standing nearby, killing him. The basket went overboard and sank with the employee still tied off to the basket. His body was recovered the following day.

At the time of the incident, the crane was secured by the brake in the up-right position with the boom several feet from the stop pads. The operator was standing near the controls, not operating the crane, when suddenly the boom collapsed.

Investigations are underway to determine the cause. Both Life Capsules on the rig will be replaced. One of the capsules was damaged in the incident and the other capsule will be replaced due to questionable condition.
