Near-Miss Results from Failure to Sign Off on PTW

A permit to work was filled out to perform a rig service and after being signed by the RM, was taken to the doghouse. It had not been signed by the company man, so the work began without his authorization.

The derrickman climbed the mast and was greasing the crown when he accidently dropped the grease gun, which fell to the rig floor narrowly missing the AD. The RM and the Company man were both on the floor at the time and were not aware that anyone was working above.

This incident had the potential to be fatal, if not cause serious injuries. As a result:

• The incident was discussed with all crews.

• Sign ‘Man working In Mast’ is now in use.

• PTW information was made available for the crews.

• The grease gun was modified to make its use at heights safer.

• Obtain heights kits for hand tools and put into use.
