Helicopter Lifts Off Without Warning

During crew change, a helicopter prepared to disembark after loading passengers. The Helicopter Landing Officer (HLO) was assisting a seated passenger engage his safety belt when the helicopter abruptly took off, without any radio notification or warning, and proceeded to lift off the deck.

Fortunately the HLO was able to slide the door closed and quickly step out of harm’s way. And although the HLO attempted to contact the pilots immediately after the incident, there was no reply back.

Root cause was attributed to insufficient pilot training, poorly defined cabin duties that resulted in confusion; inaccurate assessment of the situation by the pilot, and failure to notice the “open door” light.
