Look Around Heavy Equipment Before Starting Work

An unfortunate accident reinforces the importance of checking around heavy equipment before starting…and ensuring crew members are fully aware of potential safety hazards.

In this instance, the crew had taken a smoking break and the victim, possibly a recent hire, located himself behind an excavator during break. He may not have noticed that work had resumed until too late; there are indications he tried to draw attention by knocking on the back of the excavator when it started operating.

Speculation is that the victim was so close to the excavator that the operator was unable to see him in the mirrors. The victim tried to jump away when excavator starting backing up, but fell in its path.

What was the cause of this accident? Complacency? Ignorance? Lack of supervision? Lack of awareness of the danger inherent in heavy equipment?No matter which; the solution lies in addressing all these possible causes through preventive measures.

Lessons learned:

  • Forbid personnel to enter the working radius of heavy equipment.
  • Assign banksmen dressed in reflective jackets to help the operator at all times
  • Perform a risk assessment each time before work begins
