Proforma PSI…when performance counts.

Before You Begin Work

To help eliminate injuries and incidents, Proforma Safety recommends the following steps be part of your Control of Work process.

  1. Determine the work to be done. Identify and assess hazards. Create the permit to work and certificates if there are special considerations.

  2. Define the controls that will be used to manage the hazards. Visit the worksite. Review the work and the draft permit.

  3. Review any work that is being conducted concurrently. Ensure workers are competent to do the job. Review necessary isolations.

  4. Inspect the worksite once again for any changes. Put the controls in place for lockout, physical isolations, etc. Ensure you have the right tools.

  5. Verify the controls are in place. Discuss work once more with the people who will do the work. Ensure authority issues the permit to work.

  6. Perform toolbox safety review and start the job. When job is complete, reinstate the plant and remove isolations if part of the work plan.

  7. Clean worksite. Sign permit as complete and return. Have authority inspect the work and the worksite. Log permit for future reference.
