Lack of Formal Rig Move Process, Communications, Cause Run-Aground Incident

A semi submersible drill rig was moving with a BOP stack suspended in the water column just above the sea floor. Suddenly the ROV and BOP ran aground; burying the stack in several feet of mud. In addition to having to remove the mud and repair the BOP, operators discovered the subsea pump umbilical had been displaced laterally by the BOP stack. Fortunately, there was no damage to the umbilical.

Subsequent situation analysis uncovered several significant causes for the accident, primary among them inattention and lack of awareness; inadequate planning of the rig move, absence of a documented process for when a formal rig move plan is required, or what is to be considered in the rig move plan. It was learned that most of the involved staff were unaware of the specific subsea infrastructure in the area.

In addition, the ROV was not positioned to enable the crew to view the BOP during the move, and the actual BOP depth had not been calculated. Complicating the situation further was inadequate communication among the various parties involved.
