Anti-Bird Nest Feature Incorrectly Installed; Brake Failure Results

A disc brake kit with anti-bird nest feature incorrectly installed on a Mobile Offshore Drilling Unit (MODU) was implicated in a braking failure, in which the traveling blocks, riser and Blowout Preventer fell 50 metres until the Top Drive impacted the riser gimbal at the rig floor level.

What happened? The MODU had recently undergone a drawworks brake upgrade (band brakes to disc brakes). Incorrectly installed anti-bird nesting components were restricting the normal 1200 psi required to operate the service brake to 200psi.

The cause of the failure was determined to be the incorrect installation of the anti-bird nest / disc brake hydraulic interface; inadequate commissioning of the upgraded disc brake system; inadequate handover of the disc brake system from the supplier to rig personnel;ineffective Management of Change process during system upgrade, and inadequate follow up and understanding of previous disc brake incidents.
